Sinai Health is looking for aClinical Nurse Specialist – Wound Care to support ourProfessional Practice Nursing team.
The Clinical Nurse Specialist (CNS) role utilizes a graduate educational preparation, in-depth nursing knowledge and expertise in meeting the health needs of individuals, families, groups, communities and populations. It involves:
- Analyzing and synthesizing knowledge;
- Understanding, interpreting and applying nursing theory and research
- Developing and advancing nursing knowledge and the profession as a whole providing leadership and promoting excellence in patient focused clinical care in collaboration with the patient, family and interprofessional team(CNA, 2008)
As a member of the interprofessional team, the Clinical Nurse Specialist (CNS) in Wound Care will function as a specialized hospital wide clinical resource, who provides expert clinical consultation (assessment, collaboration, education, interventions), and care planning for complex wounds, ostomies and continence. The CNS is expected to provide leadership, mentorship, education and support to the nursing, medical and health discipline staff. As a member of the Professional Practice and leadership teams, the CNS will also lead evidence based practice for our clinical teams, and the development of standards and quality initiatives enhancing safe high quality patient and caregiver centred care.
In this role you will:
Perform comprehensive assessments to identify symptoms, underlying health conditions, physical, functional, emotional and cognitive status, home environment issues and home supports needed for patients with complex wounds and ostomies admitted to Hennick Bridgepoint Hospital.
- Analyze patterns of patient responses and needs within a complex population and develops strategies to create unit, organization and system based changes to optimize wound and ostomy healthcare outcomes
- Coordinateand leads innovative training programs for family and/or caregivers of patients with ostomies and complex wounds
- Provideadvanced practice clinical consultation and support to the organization regarding wound and ostomy issues
System Leadership:
- Facilitateknowledge translation in the clinical setting to support care in highly complex and unpredictable situations
- Supportnursing practice in meeting the standards of care for patients living with ostomies and complex wounds through mentoring, role modelling, consultation and education
- Facilitatethe strategic and innovative program planning and evaluation of wound and ostomy care
- Incorporateand disseminates knowledge from continuous learning to advance and evaluate nursing practice
- Promotethe role of wound and ostomy nursing through involvement in academic pursuits, professional associations and special interest groups
- Liaisewith academic partners to develop and provide wound and ostomy care educational programs
- Perform other duties consistent with the job classification as required
Evaluation and Research:
- Critique, interpret, synthesize, utilizeand disseminateevidence as it relates to wound, ostomy and continence nursing, quality improvement and patient safety initiatives
- Useresearch and outcome data to formulate, evaluate and/or revise policies, procedures and guidelines, standards of care and programs
- Developand evaluatewound and ostomy programs and services at all levels of the organization.
- Act as a principal investigator or collaborator with other health care team members to identify, conduct or support research that enhances wound and ostomy nursing care
- Identifythe need for practice improvement to promote safe, effective and reliable care that has a positive impact on patient and system outcomes
- Usea wide range of strategies to encourage clinicians to engage in clinical inquiry
- Apply theories and principles of safety, project management and change management
- Develop and maintain standards of nursing practice and management of patient care through creation of an environment conducive to growth and development of staff
- Participate in recruitment and retention activities
Job Type: Temporary Full Time (up to 9 months)
Job Requirements
Mandatory
- Current certificate of registration with the College of Nurses of Ontario (CNO) in good standing
- Successful completion of a Master’s degree in nursing or related field from a CNOaccredited academic institution
- Relevant specialty certificate or completion of Wound Care Education programs e.g. Wound, Ostomy and Continence CNA Certification; International Interprofessional Wound Care Course (IIWCC-CAN); Wound, Ostomy and Continence Education Program (WOC-EP); Wound Healing Master’s Program
- 3-5 years of clinical experience in wound care
- Experience in developing, monitoring, and evaluating care plans created in collaboration with the interprofessionalteam;Program planning and evaluation methodology skills
Preferred
- Member of Registered Nurses’ Association of Ontario (RNAO)
- CNS or Advance Practice Nurse experience
- Eligible for cross-appointment to an academic institution
Skills and Knowledge
- Expert knowledge in a clinical nursing specialty
- Knowledge of adult learning principles, curriculum development, and teaching learning strategies
- Demonstrated experience with knowledge
