About Las Colinas Post Acute
Located in Ontario, California, Las Colinas Post Acute is a Medicare- and Medicaid-certified skilled nursing facility serving San Bernardino County. We specialize in short- and long-term care, rehabilitation, and respite services in a safe and welcoming environment. Our facility features spacious rooms, state-of-the-art equipment, and a dedicated team of professionals committed to helping residents regain independence and quality of life
What You’ll Do
- Lead and manage the rehabilitation department across all therapy disciplines (except PTA)
- Oversee therapy operations, staffing, scheduling, and compliance
- Collaborate with interdisciplinary teams to ensure patient-centered care
- Monitor clinical outcomes, documentation standards, and productivity
- Foster a positive, growth-oriented culture within the rehab team
What We’re Looking For
- Licensed OT, COTA, PT, or SLP in California
- Previous leadership or supervisory experience in a skilled nursing or rehab setting preferred
- Strong communication, organizational, and interpersonal skills
- A passion for patient care and team development
